Job Description

At Resilient Healthcare, we are seeking an experienced Advanced Practice Provider to join our team of healthcare professionals. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality patient care in the comfort of their own homes.

Key Responsibilities:

  • Meet with new patients to orient them about services, identify needs, and suggest physicians and services.
  • See hospital at home patients for laboratory testing, examination, diagnosis, and treatment of chronic and acute episodic illnesses and minor injuries.
  • Perform physical examinations and conduct support groups in subjects related to physical and emotional health.
  • Act as a resource person for decisions, planning, and policies affecting the health of patients.
  • Maintain patient records and compile reports concerning care at patient's home.

Requirements:

  • Master's Degree in Science of Nursing or Physician Assistant Studies or related required.
  • Minimum 3 years of advanced practice provider experience required.
  • Advanced Practice Registered Nurse License or Physician Assistant License in the State of Texas required.
  • Family Nurse Practitioner or Adult Gerontology or Acute Care Nurse Practitioner certification or Physician Assistant Certification from National Commission on Certification of Physician Assistants required.
