The OpportunityThe role of the Nurse Practitioner, Virtual Resource Center is a critical position for the Wentworth Health Partners team. This position is responsible for supporting the care of our primary care patients.
We are seeking a dedicated and experienced Nurse Practitioner to see patients in-person and virtually, remotely manage Epic in-baskets, and coordinate care for patients of all ages.
Key Responsibilities- Caring for primary care patients both in-person and virtually
- Responding to patient portal messages in a timely manner
- Assisting primary care clinicians with Epic In-Basket management
- Evaluating incoming/routed patient questions and, when urgent, creating a telemedicine visit
- Diagnosing and treating patients via telemedicine, including recommending suitable treatment plans and considering cost-effective treatment modalities
- Collaborating with clinicians in managing acute and long-term medical needs of patients
- Maintaining continuity of care between physician visits
- Maintaining and reviewing patient records, charts, lab results, and other pertinent information
- Providing medical and wellness education
- Prescribing medication, including support of refill pools, in accordance with state statutes and professional practice guidelines
- Collaborating with specialty care as appropriate
- Reviewing/managing incoming reports (e.g. lab, imaging)
- Participating in peer review, quality assurance, provider meetings, and other clinical meetings
- Assisting in updating/implementing protocols and principles of practice as requested
This is a full-time, 40-hour per week hybrid position. The Nurse Practitioner will work Monday-Friday, 8:00am to 5:00pm, splitting the week on-site versus remotely. We are located at 67 Corporate Drive Building A, Portsmouth, NH 03801.
