Bayhealth is seeking a skilled Nurse Practitioner to join our team. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with other healthcare professionals to achieve optimal patient outcomes.
Key Responsibilities:
1. Conduct thorough patient assessments, develop and implement individualized care plans, and provide evidence-based treatment to patients with various medical conditions.
2. Collaborate with physicians and other healthcare professionals to ensure seamless patient care and coordination of services.
3. Provide education and counseling to patients and their families on health promotion, disease prevention, and management of chronic conditions.
4. Maintain accurate and confidential patient records, adhering to federal and state laws and regulations.
5. Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote excellent patient care.
Requirements:
1. Master's degree in Nursing or related field from an accredited institution.
2. Current Delaware State Certification/License as a Nurse Practitioner and Advanced Practice RN - Controlled Substances Registration.
3. Basic Life Support certification and two years of experience as a Nurse Practitioner in a physician practice setting or related nursing experience.
Preferred Qualifications:
1. Advanced Oncology Certified Nurse Practitioner certification.
2. Three years of experience as a Nurse Practitioner in a specific area of practice, such as oncology.
