About the JobThe successful candidate will work closely with patients, families, and multidisciplinary teams to coordinate care activities, facilitate communication, and promote effective discharge planning.
As a key member of the care delivery team, you will be responsible for:
- Coordinating patient care activities, including education and support services
- Facilitating communication between patients, families, and healthcare providers
- Developing and implementing individualized care plans to meet patient needs
- Maintaining accurate records and reporting on patient progress
To be considered for this opportunity, applicants must possess:
- A degree in nursing (BSN preferred)
- Licensure as a registered nurse in the state of Michigan (must obtain within 90 days of hire)
- Excellent communication, organizational, and analytical skills
- Ability to work collaboratively as part of a dynamic team
