Salary & Benefits

The salary for this position ranges from $23.24 to $28.28 hourly, depending on experience. As a valued member of our team, you will also enjoy a comprehensive benefits package, including medical, dental, and vision coverage, as well as paid time off and holidays.

About the Role

We are seeking a skilled Case Manager to join our Intensive Case Management Services (ICMS) Program. In this role, you will provide field-based direct client services to program participants, helping them achieve stable housing and improve their overall quality of life.

  • Responsibilities:
  • Provide case management services, including independent living skills, housing stabilization, money management, community integration, employment linkage, benefits establishment, community integration, and linkage to community providers for substance use, primary, and mental health care
  • Collaborate with clients to develop individualized treatment plans that address short- and long-term goals
  • Provide ongoing case management support to assess plan progress and ensure goals are met or adjusted as necessary
  • Conduct home visits, outreach, and engagement activities to promote participant success
  • Maintain documentation standards as set forth by the program contract and PATH policies
Requirements

To be successful in this role, you must have:

  • Maintained and executed confidential information according to HIPAA standards
  • Demonstrated knowledge of evidence-based case management techniques, including critical time intervention, trauma-informed care, harm reduction strategies, crisis intervention techniques, and motivational interviewing
  • Possessed a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
  • Worked effectively independently and as part of a team
  • Exercised mature judgment and were highly motivated, self-starting, and proactive
  • Been an excellent communicator, both written and verbal
  • Had strong prioritization skills and been able to coordinate multiple demands in a high-pressure environment
Bonus Qualifications

While not required, candidates with two years of experience in healthcare, case management, homeless services, or working with vulnerable populations will be considered preferred candidates.
