Homeless Services Case Manager Position

About the RoleThis is an exciting opportunity for a skilled and empathetic Case Manager to join our team in Los Angeles. As a member of our ICMS (Intensive Case Management Services) program, you will play a critical role in supporting individuals with complex needs and advocating for change in the community.
Salary and BenefitsThe salary for this position ranges from $23.28 to $28.28 per hour, with 24 pay periods per year. We offer a comprehensive benefits package, including medical, dental, and vision coverage, vacation and sick time, paid holidays, and a retirement plan.
Job Responsibilities- Provide field-based, direct client services to program participants, including independent living skills, housing stabilization, money management, community integration, employment linkage, benefits establishment, community integration, and linkage to community providers for substance use, primary, and mental health care.
- Collaborate with clients to develop individualized treatment plans that address short- and long-term goals.
- Provide on-going case management support to assess plan progress and ensure goals are met or adjusted as necessary.
- Maintain documentation standards as set forth by the program contract and PATH policies.
- A minimum of two years of experience in healthcare, case management, homeless services, or working with vulnerable populations or a related field.
- Excellent communication, organizational, and problem-solving skills.
- A high level of tolerance and understanding for individuals with urgent and multiple case management and health needs.
- The ability to work independently and collaboratively as part of a team.
