Job Summary

We are seeking a highly motivated and experienced RN Case Manager to join our team. As a RN Case Manager, you will be responsible for providing skilled nursing care, case management, and support to terminally ill patients and their families in a home or facility setting.

Key Responsibilities
  • Complete initial evaluations and re-evaluate patient nursing needs during each visit
  • Assume primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating patient plans of care
  • Provide appropriate support at time of death and perform bereavement assessments if requested
  • Communicate effectively with physicians, patients, families, facilities, visitors, and other organization personnel
Requirements
  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
  • Current State License as a Registered Nurse
  • Minimum one year experience as a nurse in a clinical care setting
  • Nursing skills as defined by accepted nursing standards
  • Ability to assess and respond to patient needs, function effectively in home or nursing facility environment, and handle multiple priorities and deadlines
Preferred Qualifications
  • BSN preferred
  • Experience as a nurse in a hospice or home health environment preferred
  • Familiarity with Home Care Home Base (HCHB)
