Nurse Case Manager

Join our team of dedicated healthcare professionals at St Croix Hospice as a Registered Nurse - Hospice. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support to individuals and their families.
Job Summary:We are seeking a compassionate and skilled Registered Nurse to join our hospice team. The ideal candidate will have experience in hospice care and a strong commitment to delivering patient-centered care. As a Registered Nurse - Hospice, you will be responsible for assessing patient needs, developing and implementing individualized care plans, and providing emotional support to patients and their families.
Responsibilities:- Conduct comprehensive assessments of patients and their families to identify needs and develop individualized care plans.
- Provide high-quality patient care, including administering medications, wound care, and other treatments as prescribed by the physician.
- Collaborate with the interdisciplinary team to develop and implement care plans that meet the unique needs of each patient.
- Provide emotional support and education to patients and their families, addressing their physical, emotional, and spiritual needs.
- Document patient progress and communicate effectively with the healthcare team to ensure seamless care coordination.
- Graduate of an accredited school of nursing with a current RN license in the state of employment.
- One to two years of recent acute care experience in an institutional setting, with one to two years of recent experience in hospice care.
- Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
- Prolonged or considerable walking or standing, able to lift, position, and/or transfer patients, and able to lift supplies and equipment.
