About This Role

This position provides comprehensive case management of an allocated caseload, including a range of therapeutic interventions, education, counseling, and care coordination with other services.

This role reports to the Clinical Nurse Specialist – Community Mental Health.

Key Responsibilities
  • Provide case management services to mental health consumers, their carers, and families
  • Develop and implement individualized treatment plans
  • Collaborate with multidisciplinary teams to ensure comprehensive care
  • Provide education and support to consumers and their families
  • Coordinate care with other services and agencies
Requirements
  • Eligibility for registration as a Registered Nurse with the Nursing and Midwifery Board of Australia
  • Experience in mental health nursing, preferably in a community setting
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Knowledge of mental health legislation and policies
