About the Role:
We are seeking a highly skilled and compassionate Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) to join our team at Medix™ as a Care Management Specialist. In this role, you will work closely with our integrated primary care and behavioral health team to provide exceptional care to our patients.
Key Responsibilities:
- Function as the Care Manager within the integrated primary care and behavioral health team, working closely with health consultants and primary care providers to enroll eligible patients into our Circle of Care project and track patient progress towards grant outcomes.
- Conduct face-to-face visits and telephone calls with Circle of Care patients and other members of their network, as clinically appropriate, to provide ongoing support and guidance.
- Establish and maintain a patient registry to monitor clinical outcomes and key processes of care, ensuring seamless coordination and communication with our care team.
- Arrange for inpatient, partial hospitalization, and intensive outpatient programs as needed, and assist with discharge coordination to ensure a smooth transition for our patients.
- Attend care team meetings and document clinical information and decision-making in the patient registry, ensuring accurate and up-to-date records.
- Identify barriers to treatment and possible solutions, including coordinating transportation when needed, to ensure our patients receive the support they require.
- Establish and maintain cooperative referral relationships with community agencies, organizations, and professionals, maintaining an up-to-date referral listing to ensure our patients receive the best possible care.
- Facilitate referrals to other outpatient therapists and psychiatry services as indicated, ensuring our patients receive comprehensive care.
- Participate in staff in-service programs related to social services and behavioral health, staying up-to-date on the latest best practices and research.
- For LCSWs only, provide clinical supervision for licensure to LMSWs, supporting the growth and development of our team members.
Periodic Duties:
- Contribute services to community public health activities as appropriate, leveraging our expertise to drive positive change in our community.
- Participate in Center work teams and staff problem-solving groups, collaborating with our team to drive innovation and excellence.
- Perform other duties as assigned, embracing the dynamic nature of our work and the needs of our patients.
Working Relationships:
Inside Center: All Inclusive.
Outside Center: Patients, community organizations and agencies, professional colleagues and organizations, and institutions of learning.
Qualifications:
- MSW required; LMSW or LCSW preferred.
- Minimum of three (3) years experience working in the field of social work or behavioral health care is required.
- Work experience or strong desire to work in an urban community health setting is preferred.
