Key Responsibilities
  • Assessment and Coordination: Conduct thorough assessments of patients' medical needs and develop personalized care plans in collaboration with the healthcare team.
  • Skilled Nursing Care: Provide skilled nursing care, including wound care, IV therapy, and medication management, to patients in their homes.
  • Communication and Coordination: Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination and timely updates on patient status.
  • Documentation and Reporting: Accurately document patient information, progress, and care plans in accordance with regulatory requirements and SSM Health policies.
  • Collaboration and Teamwork: Work collaboratively with the interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive patient care.
Requirements
  • Education: Graduate of an accredited school of nursing.
  • Experience: No experience required, but prior home health or hospice experience is preferred.
  • Licensure: Current RN licensure in the state of employment and Basic Life Support (BLS) certification.
  • Physical Demands: Ability to lift, carry, and push/pull objects weighing up to 25 pounds, and to stand, walk, and sit for extended periods.
