Job Summary

We are seeking a skilled and detail-oriented Case Management Assistant to join our team at MaineHealth. In this role, you will work closely with RN Case Managers, Social Workers, and Clinical Documentation Improvement Specialists to provide exceptional patient care and support.

Key Responsibilities

  • Coordinate and oversee the day-to-day operations of the Case Management Department, ensuring seamless communication and collaboration with care providers and physicians.
  • Record and transmit information necessary to support resource management of patients, utilizing strong organizational and technical skills.
  • Provide exceptional customer service and communication skills, working productively in a team structure and capable of handling multiple tasks simultaneously.
  • Utilize Microsoft Office products proficiency to manage and analyze data, and possess basic medical terminology knowledge.
  • Develop and maintain relationships with community agencies and resources, supporting the needs of patients and families.

Requirements

  • Minimum of two years' experience in a medical setting, with Associates degree preferred.
  • Strong customer service skills, good verbal and written communication skills, and ability to work in a fast-paced environment.
  • Proficiency in Microsoft Office products, with some knowledge of community agencies and resources.
