Job Summary

We are seeking a highly skilled Quality Improvement Registered Nurse to join our team at CommuniCare Health Center. As a key member of our Quality Management Department, you will play a vital role in ensuring the highest standards of quality care are delivered to our patients.

Key Responsibilities
  • Collaborate with the Director of Quality to develop and implement a systematic program for outcomes management and quality improvement efforts.
  • Ensure compliance with internal and external regulatory reporting requirements, reviewing employee immunization records and maintaining OSHA exposure files.
  • Work closely with clinical staff to optimize workflows for primary care practices transitioning to value-based, population health payment models.
  • Conduct research, collect and analyze data reports, and implement processes to achieve improved performance.
  • Prepare and present summary reports and trend analyses of program activities to the QA/PI committee.
  • Facilitate and lead organizational improvement activities using the PDSA improvement methodology.
  • Organize and facilitate meetings and special events for the Quality Management Department.
  • Provide excellent internal and external customer service.
  • Maintain ongoing professional growth and development.
  • Ensure confidentiality and maintain patient, staff, and agency information.
Requirements
  • Associate's degree in nursing required; Bachelor's degree preferred.
  • Licensed as a Registered Nurse in the state of Texas.
  • Minimum three years' experience as an RN required; minimum three years' experience in Quality Assurance in healthcare settings required.
  • Must achieve Quality Improvement certification within two years of accepting the position, if not already obtained.
  • Bilingual in English and Spanish preferred.
  • Computer literacy with extensive knowledge of spreadsheets and graphs.
  • Reliable transportation to perform clinical site audits.
Physical and Mental Requirements
  • Finger dexterity and normal fine and gross motor control of fingers and hands.
  • Talking and hearing abilities.
  • Repetitive motions and visual acuity.
  • Ability to sit and stand for extended periods and lift up to 25 pounds.
