Job Summary:
The People Concern is seeking a highly skilled Clinical Case Manager, Outreach to join our multidisciplinary street outreach team in Santa Monica. As a key member of our team, you will work closely with clients to develop individualized service plans, engage them in public benefits, and connect them with appropriate medical and behavioral health treatment.
Key Responsibilities:
- Provide field-based services to meet clients where they are, eliminating barriers to services.
- Assess mental health service needs and engage clients in developing mutually agreed-upon goals.
- Provide risk assessment and clinical evaluations for clients who are potentially gravely disabled.
- Link clients with medical, mental health, substance abuse, and vocational resources.
- Assist clients in obtaining all benefits to which they are entitled.
- Build relationships with County mental health and other resources to increase collaboration and responsiveness to clients with severe mental illness and addiction.
- Complete all charting in a timely manner.
- Collaborate and communicate effectively with police officers and local hospital staff.
Qualifications:
- Master's degree or higher in a mental health field (MSW, MFT, PhD, PsyD).
- Demonstrated knowledge of clinical case management.
- Experience in dealing with issues faced by the population served (serious mental illness, substance abuse, and physical health problems).
- Excellent writing and charting skills.
- Knowledge of community resources.
- Able to maintain an effective working relationship with Santa Monica community partners.
