About the JobWe are seeking an experienced Registered Nurse to join our team as a Chronic Disease Management Outcomes Coordinator. In this role, you will be responsible for providing care coordination across the continuum, developing and implementing individualized care plans, and monitoring patient progress to ensure optimal health outcomes.
Key Responsibilities- Under the supervision of a Chronic Disease Management Provider, provide care coordination for patients with chronic diseases.
- Develop and implement individualized care plans that promote self-management, health education, and disease prevention.
- Monitor patient progress and adjust care plans as needed to ensure optimal health outcomes.
- Maintain accurate and up-to-date patient records in accordance with regulatory requirements.
- Participate in quality improvement initiatives to enhance care coordination services and improve patient satisfaction.
- Current Alaska Registered Nurse License or ability to obtain within six months of hire.
- Two years recent nursing experience, with case management experience preferred.
- Graduate of an Accredited School of Nursing.
