About the Role

We are seeking an experienced Registered Nurse to join our team as a Chronic Disease Management Outcomes Coordinator. This role is responsible for providing care coordination across the continuum, facilitating communication between healthcare providers and patients, and monitoring patient condition to ensure optimal health outcomes.

Key Responsibilities
  • Under the supervision of a Chronic Disease Management Provider, provide care coordination for patients with chronic diseases.
  • Facilitate collaboration between healthcare providers and patients to develop comprehensive care plans.
  • Collect and analyze data on patient outcomes to identify trends and areas for improvement.
  • Maintain accurate and up-to-date patient records in accordance with regulatory requirements.
  • Participate in program development and continuous improvement initiatives to enhance care coordination services.
Requirements
  • Current Alaska Registered Nurse License or ability to obtain within six months of hire.
  • Two years recent nursing experience, with case management experience preferred.
  • Graduate of an Accredited School of Nursing.
