Job Definition:

The OnTrackNY Program is a Coordinated Specialty Care (CSC) treatment program at CN Guidance and Counseling Services, which serves adolescents and young adults experiencing first episode psychosis.

The Primary Clinician works closely with the team to develop a positive clinical relationship with clients and facilitates linkage with community supports.

Education Requirement:

Licensed Clinical/Masters Social Worker, Licensed Marriage and Family Therapist, or Licensed Mental Health Counselor

Experience Required:

1-year experience in mental health and/or chemical dependency community-based setting

Duties and Responsibilities:
  1. Primary resource for participants and their families to provide emotional and practical support, focusing on participant outreach, engagement, and retention.
  2. Conducts needs assessments, provides psycho-education, and engages in safety planning and wellness management planning.
  3. Conducts clinical assessments to determine the participant's level of social functioning and substance use via a shared decision-making framework.
  4. Draws on expertise of other team members and deploys team resources.
  5. Gathers sufficient information for each individual to make a diagnostic assessment necessary to determine assignment priorities and treatment plan.
  6. Engages in safety planning and wellness management planning with participants and families.
  7. Connects participants and families with the services and supports they need.
  8. Provides behavioral interventions and takes an especially active role in the family support domain.
  9. Provides Social Skills Training, Substance Abuse Treatment, Behavioral Activation, Coping Skills Training, and Psycho-education.
  10. Provides psychoeducation regarding first episode psychosis (FEP) and the goals of the OnTrackNY program.
  11. Provides supportive therapy.
  12. Develops and modifies treatment using a shared decision-making process with clients.
  13. Works with participants and families for transition from the team to other services and supports, and ensures stable links at discharge.
  14. Rotating on-call responsibilities with other team members.
  15. Engages in performing assessments, handling crisis intervention, and discharge.
  16. Provides direct, face-to-face and/or collateral services to individuals either in the OnTrackNY office or in the community.
  17. Regularly assesses progress with Team Leader.
  18. Completes QI Training during onboarding and participates in QI refresher training as needed.
  19. Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views, and experiences.
  20. Identifies and implements quality improvement trainings and activities as part of professional development goals.
  21. Encourages quality improvement processes and provides consistent resources for QI and professional development.
  22. Adheres to all safety protocols and procedures to ensure a safe working environment for all employees.
