Qualifications
Bachelor's Degree in mental health, healthcare, or a related Human Services discipline. Candidates with a Bachelor's Degree in other fields must possess at least two years of experience in a community behavioral healthcare environment. High School Diploma or GED along with relevant life experience is also acceptable in lieu of a Bachelor's Degree.

Responsibilities
  • Deliver outreach services utilizing strengths-based, person-centered approaches.
  • Facilitate outreach, engagement, case management, and skill-building initiatives.
  • Manage cases and provide ongoing connections to community resources.
  • Encourage the integration of physical and mental health services.
  • Offer 24/7 support to program participants through an on-call rotation.
  • Assist individuals in accessing community resources that align with their needs.
  • Ensure timely completion of all required documentation in clinical files and in HIS, adhering to policies and regulations.
  • Participate in supervision and agency staff meetings.
  • Engage with program partners and community providers to achieve program goals.
  • Meet expected productivity levels concerning direct service delivery.
  • Perform additional duties as assigned by the Program Director.
