About the Role:

We are seeking a skilled Healthcare Coordinator to join our team at Duke University. In this role, you will play a vital part in coordinating internal and external referrals for Duke HomeCare & Hospice, as well as providing initial intake of provider orders.

Key Responsibilities:

  • Provide nursing care for patients in accordance with physician's order and established policies and procedures
  • Participate in weekly team meetings and case conferences, as necessary, to facilitate that appropriate services are provided to the patient
  • Maintain up-to-date knowledge of Medicare/Medicaid/JC through in-service and workshop attendance
  • Coordinate care plan activities with multidisciplinary team members as directed by Nursing Case Manager
  • Document care and services provided in accordance with DHCH policies, procedures, and regulatory standards

Requirements:

  • Graduation from an accredited Practical Nurse program
  • One to two years of experience preferred
  • Working knowledge of Medicare and Medicaid regulations
  • Strong Computer skills preferred

Salary Information:

The estimated salary for this position is $60,000 - $80,000 per year, depending on experience and qualifications.

Location:

This position is located in Durham, NC, with opportunities for hybrid work arrangements.
