About the Role

We are seeking a skilled Licensed Practical Nurse to join our team at TRC Total Renal Care Inc. as a Renal Care Specialist. As a key member of our dialysis care team, you will play a vital role in delivering high-quality patient care and contributing to the success of our organization.

Key Responsibilities
  • Provide direct patient care in an outpatient dialysis setting, working closely with patients and their families to ensure optimal health outcomes.
  • Collaborate with healthcare professionals to develop and implement individualized treatment plans, taking into account patients' unique needs and medical histories.
  • Conduct thorough assessments, administer medications, and perform various medical procedures as required.
  • Monitor patients' progress, identifying and addressing any concerns or complications in a timely and effective manner.
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices and policies that enhance patient care and safety.
Requirements
  • Current LPN-LVN license in the state of practice required.
  • Current CPR certification required.
  • IV certification required if required by state regulations.
  • High school diploma or equivalent required.
  • Completed training program approved by the medical director and the governing body as required by CMS guidelines prior to operating the water treatment system.
  • BONENT or CHT certification preferred or as required by state regulation.
  • Minimum of one year's experience in a hospital or clinical setting preferred.
  • Basic computer skills and proficiency in MS Word and Outlook required; functional proficiency in computer systems of DaVita within 90 days of hire required.
