Pharmacist - Transitions of Care

Brigham and Women's Hospital (BWH) is seeking a highly skilled and experienced Pharmacist to join our team as a Transitions of Care Pharmacist. Key Responsibilities- Prepare, compound, and dispense medications to patients according to laws and regulations governing pharmacists and pharmacy practice.
- Optimize drug therapy by identifying, resolving, and preventing drug-related problems and improving therapeutic outcomes.
- Work independently and within established pharmacy/ambulatory care practice site policies and procedures as a member of the patient care team.
- Direct and coordinate the activities of a pharmacy service area, supervising support personnel and ensuring the accuracy of all work completed.
- Provide direction and guidance to pharmacist and ancillary support staff, promoting professional behavior and attitudes.
- Participate in resolving personnel management issues and provide feedback to pharmacy managers/practice managers/medical directors for performance appraisals.
- Direct and indirect patient care services, including preparing, checking, and dispensing medications, and overseeing the preparation of sterile and non-sterile compounds.
- Interpret and implement departmental policies and procedures, maintaining expertise in pharmacy practices and regulatory requirements.
- Ensure compliance with target drug programs, formulary management, medication management initiatives, and adverse drug event monitoring programs.
- Document and report clinical interventions, medication errors, and adverse drug events.
- Participate in department quality improvement efforts and maintain skill and competency levels to comply with departmental and hospital quality assurance standards.
- Bachelor of Science degree in Pharmacy or Doctor of Pharmacy from an accredited college of pharmacy.
- Eligibility for licensure to practice pharmacy in the Commonwealth of Massachusetts.
- Post-graduate residency/fellowship training preferred.
- Board Certification by examination in a specialty preferred.
- Completion of 20 continuing education credits per year to maintain licensure.
- Annual completion of institutional and pharmacy department competency assessments.
- Exceptional oral and interpersonal communication skills.
- Ability to plan and coordinate activities, prioritize tasks, and respond positively to new approaches and changes in the work environment.
- Extensive knowledge and ability in all areas of the department, including IV Admixtures, Drug Information, Ambulatory, Manufacturing, and Drug Distribution.
- Ability to train new employees and assume responsibility for coordinating pharmacy services on an assigned shift.
