Job Title: LVN Charge Nurse

We are seeking a highly skilled and compassionate LVN Charge Nurse to join our team at Heritage Park Rehabilitation and Skilled Nursing Center in Austin, TX.

Job Summary:

The LVN Charge Nurse will play a critical role in ensuring the provision of high-quality care to our residents. This includes assisting in the development and implementation of individualized care plans, administering medications, and conducting rounds to monitor resident condition.

Key Responsibilities:
  • Assist in the development and implementation of individualized care plans
  • Administer medications as prescribed by physicians
  • Conduct rounds to monitor resident condition and document changes
  • Oversee and monitor aides to ensure high-quality care
  • Initiate and complete incident and accident reports as needed
  • Communicate effectively with residents, families, and healthcare team members
Requirements:
  • Active Licensed Vocational Nurse (LVN) license in the state of Texas
  • Experience working in a skilled nursing facility preferred
  • Ability to work effectively in a fast-paced environment
  • Strong communication and interpersonal skills
Physical Demands:

The physical demands of this role include standing, sitting, walking, and lifting up to 50 pounds. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this job.
