Job Title: LVN Charge Nurse

Regency Integrated Health Services is seeking a skilled and compassionate LVN Charge Nurse to join our team at Southpark Meadows Nursing and Rehab Center in Austin, TX.

Job Summary:

The LVN Charge Nurse will play a vital role in ensuring the delivery of high-quality care to our residents. This position requires a strong leader who can effectively manage and coordinate the care of our residents, while also promoting a positive and supportive work environment.

Responsibilities:
  • Assist in the development and implementation of individualized care plans that meet the unique needs of each resident.
  • Conduct regular rounds to assess resident condition and identify any changes or concerns.
  • Administer medications as prescribed and ensure accurate documentation of resident care.
  • Collaborate with the healthcare team to ensure seamless care transitions and effective communication.
  • Monitor and oversee the work of nursing assistants to ensure high-quality care is provided.
  • Identify and report any significant changes in resident condition to the Director of Nurses and/or Administrator.
  • Participate in quality improvement initiatives to enhance the overall care experience for our residents.
Requirements:
  • Active Licensed Vocational Nurse (LVN) license in the state of Texas.
  • Experience working in a skilled nursing facility is preferred.
  • Strong leadership and communication skills.
  • Ability to work effectively in a fast-paced environment.
  • Compassion and empathy for residents and their families.
