Key Responsibilities:
  • Conduct regular rounds to assess resident condition and identify areas for improvement
  • Develop and implement care plans that promote resident well-being and independence
  • Administer medications as prescribed and monitor resident response
  • Collaborate with the healthcare team to ensure seamless care transitions and communication
  • Identify and report any changes in resident condition or status
  • Supervise and mentor nursing staff to ensure high-quality care delivery
Requirements:
  • Active Licensed Vocational Nurse (LVN) license in the state of Texas
  • Experience working in a skilled nursing facility preferred
  • Strong communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
