Position Summary
RN Care Manager, Social Services
Shift: Full Time, 8am-4:30pm
We are in search of a skilled Registered Nurse / RN Care Manager with extensive experience in patient care and a solid understanding of the needs of specific patient populations.
The RN Care Manager (RN CM) plays a crucial role in coordinating patient care from admission to discharge. This includes ensuring seamless transitions of care as patients are discharged from hospital settings, facilitating high-quality clinical outcomes, securing post-acute services, and advocating for patients and families with both internal and external stakeholders. The RN CM also identifies and addresses potential barriers to care coordination and discharge planning to promote efficient care delivery and maximize reimbursement.
The RN CM initiates care coordination upon patient admission by conducting thorough assessments, which enable timely and accurate information capture and the development of discharge plans.
As an integral member of the interdisciplinary care team, the RN CM participates in rounds, care conferences, and team meetings, representing both the hospital care team and the patient/family to balance care needs with service execution capabilities.
The RN CM collaborates with the care team and the patient/family to devise and implement optimal care plans, considering various factors, limitations, and preferences.
Additionally, the RN CM identifies and recommends post-acute services, ensuring timely referrals to appropriate providers while coordinating with patients/families and the care team.
Ultimately, the RN CM is accountable for aligning the discharge plan with the patient's medically cleared discharge date and the projected length of stay as determined by the payer.
The RN CM actively contributes to strategies aimed at reducing unnecessary length of stay and resource utilization.
In cases requiring escalation, the RN CM collaborates with management, Physician Advisors, Complex Care teams, and/or Ethics committees.
Furthermore, the RN CM educates patients/families and the care team regarding post-acute services, transitions of care, readmission prevention, appropriate care level choices, and available resources.
The RN CM provides supportive and therapeutic communication to patients, families, and loved ones facing anxiety or stress due to health challenges.
Effective communication is essential; the RN CM must convey information confidently and therapeutically while maintaining a professional demeanor that reflects positively on the organization.
Minimum Qualifications
- Valid, active, unencumbered Nursing license or temporary permit approved by the relevant Licensing Board.
- Bachelor's degree in nursing preferred or actively pursued.
- Minimum of 2 years recent healthcare experience required.
- Must meet all quality and productivity expectations and successfully complete annual competencies.

Physical Requirements
- Ability to lift 20-50 lbs occasionally; 11-25 lbs frequently; and 1-10 lbs constantly.
