Position Summary

We are in search of a dedicated Registered Nurse / RN Care Manager with extensive experience in patient care and a deep understanding of specific patient populations.

The RN Care Manager (RN CM) plays a crucial role in coordinating patient care from admission to discharge, ensuring seamless transitions as patients leave the hospital, facilitating high-quality clinical and cost-effective outcomes, securing post-acute services, and advocating for patients and families with both internal and external stakeholders. This role also involves identifying and addressing potential barriers to care coordination and discharge planning to enhance efficient care delivery and maximize reimbursement.

The RN CM initiates care coordination upon patient admission by conducting thorough assessments, enabling timely and accurate information capture, and fostering the development of a discharge plan.

As an integral member of the interdisciplinary care team, the RN CM is expected to participate in rounds, care conferences, and team meetings.

The RN CM serves as a liaison between the hospital care team and the patient/family, balancing patient/family preferences with care coordination needs.

Working collaboratively with the hospital care team and the patient/family, the RN CM will devise and implement optimal care plans while considering various factors, limitations, and preferences.

The RN CM identifies and recommends appropriate post-acute services and promptly completes referrals to relevant providers, coordinating directly with patients/families and the care team.

Ultimately, the RN CM ensures that the discharge plan aligns with the patient's medically cleared discharge date and the projected length of stay as determined by the payor.

The RN CM actively participates in developing strategies to minimize unnecessary length of stay and resource utilization.

In appropriate cases, the RN CM escalates issues to management, the Physician Advisor, the Complex Care team, or the Ethics committee.

It is essential for the RN CM to educate patients/families and the care team regarding post-acute services, transitions of care, readmission prevention, and available resources.

The RN CM provides compassionate and therapeutic communication to patients, families, and loved ones experiencing anxiety or stress due to health challenges.

Effective communication is vital; the RN CM must convey information confidently and assertively while maintaining a positive representation of the organization.

Minimum Qualifications

- Must possess a valid, active, unencumbered Nursing license or a temporary permit approved by the relevant licensing board.
- A Bachelor's degree in nursing is preferred or actively being pursued.

Experience

- A minimum of 2 years of recent healthcare experience is required.
- Must meet all quality and productivity expectations and successfully complete annual competencies.
