Clinical Pharmacist, Cancer Center Specialist

The GW Medical Faculty Associates is seeking a highly skilled Clinical Pharmacist to join our team at the GW Cancer Center. As a key member of our multidisciplinary team, you will play a critical role in providing comprehensive pharmaceutical care and disease management in the ambulatory care setting to adult patients with cancer.
Key Responsibilities- Build and manage a Clinical Pharmacist Practitioner (CPP) clinic that collaborates with physicians to see patients and bill for services
- Perform medication reconciliation for all patients seen at the clinic
- Serve as a liaison between the Department of Pharmacy and Hematologic Malignancies Clinic to ensure all activities are coordinated and consistent
- Educate patients and their families about their treatment plans and subsequent medication management
- Manage toxicities caused by chemotherapy agents for all patients seen in the clinic
- Review all chemotherapy orders written/placed by clinic physicians prior to being sent to pharmacy
- Provide coverage of the acute care services, including order verification, profile review, chemotherapy order preparation, discharge counseling, and clinical consultation
- Collaborate with the acute care pharmacist responsible for the Medical Oncology Hematology Service on policies, procedures, and guidelines associated with this patient population
- Doctor of Pharmacy degree from an accredited institution or an equivalent combination of training and experience
- Current or valid licensure as a Pharmacist in the District of Columbia or the eligibility to earn such licensure
- Board Certification in Oncology Pharmacy or another BPS specialty
