Job Summary

Under the general supervision of the Supervisor, Community Support, this role will provide case management, support, education, and consultation for individuals experiencing behavioral health concerns. The ideal candidate will be an effective team member and represent the agency in a professional manner.

Key Responsibilities
  • Provide community-based/in-home behavioral health services for individuals experiencing behavioral health concerns.
  • Coordinate with Emergency Response Service, other crisis service teams, and supervisor for all crisis situations.
  • Provide individual and group services, including monitoring of daily living activities, assistance maintaining benefits and entitlements, and support to manage symptoms.
  • Act as a liaison and resource between the community and client needs, coordinating treatment goals with community agencies, primary care physicians, court system, and managed-care entities.
  • Maintain records in accordance with UnityPlace policies, as well as state and federal oversight agencies and accreditation agencies.
Requirements
  • High school diploma or equivalent.
  • Ability to work within a provider agency's structure and accept supervision.
  • Effective written and verbal skills, allowing communication with physicians, co-workers, other clinic/hospital personnel, and the public.
