The Family Support Case Coordinator at Advocates plays a crucial role in assisting families of individuals with disabilities. This position focuses on providing education and guidance to ensure that families can effectively utilize available resources.

Educational Requirements

Key Responsibilities

  • Collaborate with families to create and execute a support plan addressing various needs, including educational, social, vocational, behavioral, and clinical requirements.
  • Assist in the development and execution of a mentoring program for adults with disabilities.
  • Compile and maintain a comprehensive list of resources available for the families we serve.
  • Guide families through public assistance systems to access benefits and resources.
  • Organize social and recreational activities for families.
  • Provide training sessions for families on benefits and transition planning.
  • Educate family members on relevant policies and procedures from state agencies, Advocates, and specific treatment approaches.
  • Offer support to family members identified by Clinical Services Directors, Program Managers, and Staff as needing assistance.
  • Ensure effective communication with staff, management, and family members.
  • Stay informed about developments in the field of disabilities and the needs of the families and individuals served.
  • Assist in organizing family committees and participate in meetings as required.

Qualifications

  • Bachelor's degree in social services or a related field with at least one year of experience working with the target population and families, or a High School Diploma/GED with three years of relevant experience.
  • Ability to perform essential duties effectively.
  • Strong written communication skills and sound judgment.
  • Proficiency in English and the ability to communicate effectively in the primary language of the assigned program.
  • Sensitivity to the needs of the population served.
  • Basic computer skills.
  • High energy, excellent interpersonal skills, and the ability to work collaboratively in a team environment.
  • Valid driver's license and access to a vehicle.
