Job Summary

We are seeking a compassionate and skilled Registered Nurse to join our Hospice team at Cantex. As a Registered Nurse, you will play a vital role in providing high-quality patient care and support to individuals and their families during their end-of-life journey.

Key Responsibilities
  • Administer nursing care to patients of all ages in their place of residence, coordinating care with the interdisciplinary team, patient, family, and referring agency.
  • Assume responsibility for the coordination of care, ensuring continuity and quality of service.
  • Conduct initial and ongoing comprehensive assessments of patients' physical, functional, psychosocial, and environmental needs.
  • Apply specific criteria for admission and re-certification to hospice care, establishing appropriate levels of care and patient eligibility.
  • Develop and implement individualized plans of care, ensuring quality and continuity of care, and recommending revisions as necessary.
  • Consult with and educate patients and families regarding disease processes, self-care techniques, end-of-life care, nutrition, and dietary needs.
  • Assess caregivers' ability to meet patients' immediate needs upon admission and throughout care.
  • Initiate preventive and rehabilitative nursing procedures, applying concepts of infection control and standard precautions.
  • Provide clinical direction to Hospice Aides and LPNs/LVNs, ensuring quality and continuity of service.
  • Ensure continuity of quality patient care, with appropriate documentation.
  • Inform physicians and other personnel of changes in patients' needs and outcomes of interventions, evaluating patient and family response to care.
  • Promote Agency philosophy and administrative policies.
  • Provide effective communication to patients, families, team members, and other healthcare professionals.
Requirements
  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • 2 years of experience as a Registered Nurse in a clinical care setting, with hospice experience preferred.
  • Nursing skills as defined by generally accepted standards of practice, with the ability to read, write, and comprehend English.
