POSITION OVERVIEW:


The Adult Residential Services division at The Jewish Board delivers compassionate, high-quality, evidence-based support to individuals and families in our communities.

Our professionals employ a culturally sensitive, person-centered methodology to assist individuals and their families in enhancing their skills and resources, fostering hope, and building resilience.

The Case Manager serves as the most experienced level of direct care staff within our supportive housing initiatives, aiding residents in achieving as much independence as possible within the community.

Manages a caseload and implements person-centered and collaborative service planning with residents.
Handles the most complex cases; maintains a larger caseload than case associates and assistants.
Ensures that service plans are completed and signed by residents in accordance with regulatory standards.

Conducts evaluations and assessments of independent living skills utilizing a detailed functional assessment tool or assists residents in utilizing the tool for self-assessment.

Aids residents in achieving their personal goals.
Supports residents in working towards medication management independence, including appropriate person-centered interventions.
Addresses crises and de-escalates situations that may threaten resident comfort and safety.
Develops management plans to enhance safety and avert ongoing crises.
Provides services to residents in alignment with program regulations and the agency's mission.
Independently maintains timely and accurate documentation of all resident interactions, including assessments, service plans, progress notes, and informational updates.
Ensures that progress notes comply with monthly billing documentation requirements, as necessary.
Files incident reports when appropriate.

QUALIFICATIONS:
The Jewish Board seeks talented professionals from all backgrounds with the following qualifications:

Required:
Bachelor of Arts (BA) or Bachelor of Science (BS) degree (At least 1 year of experience in Human Services preferred)
Exceptional resident engagement abilities; Strong verbal and written communication skills required
Meticulous attention to detail
Capacity to work independently as well as collaboratively with a team

Ability to travel to visit residents in their homes or to escort residents; Valid NYS driver's license and willingness to operate agency vehicles preferred.

Willingness to work with a seriously mentally ill population, many of whom have experienced homelessness and substance use.
Compassion and respect for vulnerable individuals
Patience and understanding for the often gradual process of stabilization and recovery
