Position Overview:
The Clinical Care Coordinator plays a pivotal role in enhancing 1) care integration, 2) care continuity, 3) attainment of expected clinical outcomes, 4) patient representation, and 5) optimal resource utilization. This position is responsible for managing the care of intricate patients from the point of pre-admission through to the immediate post-discharge phase.

Collaborative Approach:
Works in partnership with all interdisciplinary teams involved in the patient's healthcare journey, both within and outside the organization, to elevate patient care through effective resource management and monitoring. Assumes a leadership position to achieve targeted clinical, financial, and resource-related results.

Core Responsibilities:
Key functions include:
1. Assessing post-acute needs;
2. Planning transitions;
3. Coordinating care;
4. Providing education;
5. Referring to community care resources;
6. Developing care plans in compliance with regulatory standards;
7. Collaborating with the continuum care team;
8. Continuously monitoring the effectiveness of care plans and reassessing the necessity for care management services.

Interdepartmental Collaboration:
Engages with utilization management, risk management, clinical documentation improvement, and other departments to foster quality care and efficient resource management.

Communication Skills:
Effectively communicates (speaking, reading, and writing) and adeptly resolves conflicts. Utilizes strong organizational capabilities and comprehends the integration and coordination of the multidisciplinary healthcare team.

Required Qualifications:
Upon Hire:
- Valid Registered Nurse License
- Minimum of 3 years of clinical nursing experience in an acute care hospital or community setting.

Preferred Qualifications:
- Bachelor's Degree in Nursing or
- Associate's Degree in Nursing
- Care Manager Certification within 3 years of hire
- At least 1 year of experience in care management, discharge planning, or utilization management in acute, sub-acute, home health, or managed care environments.
