About the Role

Community Youth Services is seeking a dedicated and compassionate Care Coordinator to join our Transitional Age Youth team. As a key member of our treatment team, you will provide comprehensive care coordination services to youth aged 15-21 in Thurston and Mason counties.

Key Responsibilities
  • Manage a caseload of 10-15 youth, focusing on their needs and strengths.
  • Organize and lead Wraparound Team Meetings, ensuring inclusive and outcome-driven agendas.
  • Foster a supportive team culture, offering a safe space for youth and families.
  • Maintain a positive and accepting approach towards youth and family choices.
  • Exhibit leadership and conflict resolution skills to integrate diverse viewpoints.
Requirements
  • Bachelor's in Social Work, Psychology, Counseling, or a related field, or an Associate's with two years of experience in care coordination or case management.
  • Familiarity with Mental Health, Juvenile Justice, Schools, DCYF, DSHS, DVR, and community support systems.
  • Understanding of cultural issues affecting marginalized youth, commitment to Positive Youth Development and Wraparound principles, and a non-judgmental approach.
