Position Overview
The Outpatient Physical Therapy Specialist at Houston Methodist Clear Lake Hospital is tasked with evaluating patients and formulating tailored therapy plans.


This role involves educating patients, their families, and caregivers while collaborating with other healthcare professionals to resolve issues and achieve specific patient goals.

As an integral member of the patient care team, the specialist delivers direct care, ensuring adherence to safety protocols and guidelines to optimize patient safety, service quality, and overall outcomes.

Seeks input from colleagues and collaborates with all members of the interprofessional care team, effectively communicating and documenting relevant patient care information.

Serves as a resource for colleagues and may assist in mentoring students and support staff.

Patient Care

Conducts thorough evaluations of patients to assess their functional capabilities and develop appropriate treatment plans. Implements therapy plans based on evaluation results while respecting cultural diversity and inclusion principles.

Provides education tailored to the learning needs of patients and their families, regularly assessing comprehension and adjusting teaching methods as necessary.

Engages in ongoing communication with the healthcare team regarding modifications to treatment plans, patient progress, and discharge planning to meet established goals.

Advocates for patients and families to address clinical and ethical concerns, utilizing available resources effectively.

Aims to improve patient satisfaction scores through adherence to service standards and peer accountability.


Quality and Safety

Regularly assesses patients, monitors progress towards treatment objectives, and adjusts goals and plans as needed. Identifies complications or limitations related to specific procedures and collaborates with peers to ensure optimal outcomes.

Provides feedback on physical therapy-related activities performed by the interprofessional healthcare team and adjusts plans as necessary to enhance quality and maintain patient safety.

Identifies opportunities for process improvement and corrective actions, leveraging evidence-based practices to achieve patient safety outcomes, supported by accurate documentation.

Contributes to departmental and hospital quality and safety targets.


Documentation and Compliance

Maintains accurate and timely documentation of patient education and therapy procedures in patient records. Ensures accurate billing for services rendered.

Utilizes resources efficiently to optimize time and equipment usage.

Demonstrates self-motivation in managing time effectively, minimizing unnecessary overtime, and prioritizing daily responsibilities while assisting colleagues as needed.

Meets productivity standards by managing assigned patient loads effectively.


Professional Development

Engages in innovative practices through evidence-based projects and actively participates in departmental initiatives and hospital committees.

Identifies and proposes areas for improvement in patient care and operational efficiency, supported by evidence-based literature.

Seeks opportunities for continuous learning and professional growth as a Physical Therapist.

Qualifications

Education:
Graduate of an accredited physical therapy program recognized by the relevant credentialing body.

Experience:
No prior experience required.

Licenses and Certifications:
BLS - Basic Life Support (AHA)
PT - Physical Therapist - State Licensure (Compact license acceptable within 90 days)
OR
PT-Temp - Physical Therapist Temporary - State Licensure within 90 days.

Skills and Abilities:
Demonstrates the necessary skills and competencies to perform assigned duties safely, assessed through ongoing evaluations.

Proficient in English, both written and spoken, to effectively perform essential job functions, particularly those impacting patient and employee safety.

Exhibits strong communication skills with patients, families, and colleagues, emphasizing customer service and positive interactions.

Possesses competent assessment, organizational, and problem-solving abilities, capable of prioritizing multiple tasks.

Demonstrates strong interpersonal and leadership skills, ensuring excellent service delivery to patients, visitors, and colleagues.

Proficient in using various computer systems and rehabilitation equipment.

Ability to work independently and exercise sound judgment in interactions with patients and the healthcare team.

Exhibits professional and technical competence in selecting and implementing treatment plans.
