Case Manager, Community Solutions Specialist

At Community Solutions, we believe in providing second chances and empowering people to become responsible, productive citizens. As a Case Manager, you will play a key role in promoting long-term, self-sustaining skills through vocational training and personal development services.
Key Responsibilities:
Perform client needs assessments and enter data into CSI data systems.
Actively participate in Client Progress Review meetings and provide peer support for client issues.
Develop and document program plan goals with clients and monitor progress.
Facilitate individual and group sessions, including curricula facilitation and make-up sessions as needed.
Requirements:
Bachelor's degree and one year experience in case management or a similar field.
Strong decision-making, organizational, and interpersonal skills.
Previous assessment and facilitation experience preferred.
