About the Role:

We are seeking a compassionate and self-motivated Case Manager to support clients in attaining their health and housing goals. As a Case Manager, you will work with vulnerable populations in Santa Cruz County, providing high-quality case management services to clients enrolled in the L2H Program.

Key Responsibilities:

• Hold a caseload of 30-45 clients
• Assessments, goal setting, and creating and managing a care plan
• Referrals, supporting clients with health-related goals, and motivational interviewing and de-escalation
• Care coordination, including warm hand-offs and closed-loop referrals
• Train in Evidence-Based Practices by utilizing Encompass's client-centered approach
• Documentation and client advocacy

Requirements:

• 2+ years of experience working with individuals experiencing homelessness, providing housing navigation, case management services, or supporting individuals with co-occurring disorders
• Strong computer skills, including navigating healthcare systems
• Valid CA driver's license, clean driving record, and insured personal vehicle
• Preferred: BA or Masters in a related field, bilingual/bicultural
