Our Mission
- Provide community-based services to individuals on parole, helping them to build positive behaviors and contribute to their communities.
- Actively supervise community service projects, ensuring client safety and making every experience constructive and rewarding.
- Work with a dedicated team of case managers and specialists, helping clients set and achieve individual goals.
- Expand your skills through ongoing training and development in a dynamic nonprofit setting.
Responsibilities
- Perform client needs assessments within seven (7) days of intake and enter data into CSI data systems on the day the data is obtained.
- Actively participate in Client Progress Review (CPR) meetings by presenting new clients, assessments, and Individual Service Plan updates on clients as well as discharge planning for clients nearing discharge.
- Develop and document program plan goals with clients within thirty (30) days of activation and review with client on a bi-weekly basis.
- Case Managers are responsible for weekly curricula facilitation, including entering all new referrals to interventions they facilitate into the CSI data systems on the day of the referral.
Requirements
- Bachelor's degree and one year experience in case management or similar field.
- Strong decision-making, organizational, and interpersonal skills.
- Previous assessment and facilitation experience preferred.
- Experience using Microsoft Office and a commitment to working as part of a team.
