Job Summary
The Nurse Practitioner In-Home Assessment is responsible for performing comprehensive health risk assessments and transitional care for patients discharged from hospitals to prevent re-admissions.
Key Responsibilities
- Conduct comprehensive in-home health risk assessments to identify active and chronic disease conditions, as well as determine physical, mental, and social needs present at the time of the visit
- Perform follow-up visits in transitional care
Requirements
- Enrollment in both Medicare & Medicaid
