About the Position:
As a Nurse Practitioner on our Home Risk Assessment team, you will conduct comprehensive health assessments in the comfort of our members' homes. You will review their medical history, observe their current health, and provide personalized care and support to help them manage their conditions.
This is a PRN/Part Time opportunity, offering flexible scheduling and competitive compensation. You will have the opportunity to work with a diverse range of patients and develop your clinical skills in a dynamic and supportive environment.
Responsibilities:
Conduct comprehensive health assessments in members' homes
Review medical history and observe current health
Provide personalized care and support to help members manage their conditions
Collaborate with primary care providers and other healthcare professionals to ensure seamless care coordination
Maintain accurate and detailed records of patient interactions and care plans
Requirements:
Master's degree in Nursing or related field
Current RN and NP licensure in state of practice
Board certification in a Matrix-approved specialty
1 year of experience as a Nurse Practitioner
Strong computer skills and familiarity with electronic health records
Excellent verbal and written communication skills
Travel Requirements:
Ability to travel to members' homes and other locations as needed
Valid state driver's license and reliable transportation
Proof of adequate automobile insurance coverage
