Clinical Care Coordinator

Southland Healthcare and Rehab seeks a dedicated Registered Nurse to join our team.
This role requires a skilled professional who can deliver high-quality patient care and develop effective care plans under the supervision of the Director of Nursing.
Responsibilities:- Develop and implement individualized care plans for residents
- Work closely with the Director of Nursing to ensure excellent clinical operations and resident care
- A degree from an approved school for Practical or Vocational Nursing
- Current licensure by the State Board of Nursing
- Experience in geriatric or rehabilitative nursing (preferable)
