About the Role

This Case Manager position offers an excellent opportunity to make a meaningful difference in the lives of individuals with behavioral health issues.

The estimated salary for this role is around $52,000-$65,000 per year, based on industry standards and location.

Key Functions:

  1. Provide environmental support and care coordination to help individuals improve their functioning and access necessary services.
  2. Assist individuals in developing natural supports to promote community integration and identifying service needs.
  3. Refer and link individuals to services and resources identified through the service planning process.
  4. Coordinate services to maximize integration and minimize gaps.
  5. Ensure the continued adequacy of the Individual Recovery Plan (IRP) to meet ongoing and changing needs.

Qualifications:

  • A valid GA Driver's License is required.
  • A High School Diploma or equivalent document is necessary.
  • A bachelor's degree in a related field is preferred but not required.
  • Relevant work experience is desired.
  • Strong verbal and written communication skills are essential.
