Community Health Provider Job Description

We are seeking a skilled Community Health Provider to join our team at Oak Street Health. As a Community Health Provider, you will be responsible for delivering high-quality patient care to our community members. Your responsibilities will include building relationships with patients, facilitating care plans, and coordinating clinical and auxiliary services.

Your Key Responsibilities Will Include:
  • Assist the RN in patient care tasks, in accordance with the NYS LPN scope of practice
  • Collects patient healthcare data systematically and on an ongoing basis using appropriate assessment techniques and instruments
  • Administer medications and vaccinations (including injections)
  • Monitor and coordinate delivery of the care plan
  • Form relationships with patients and their caregivers to support prevention-focused care and ED/hospital diversion where appropriate
