Community Health Advocate

We are seeking a Community Health Advocate to join our team at NYC Health + Hospitals, the largest public health care system in the United States.
The successful candidate will work with patients, their families, and caregivers to provide patient-centered, community-based health services in collaboration with interdisciplinary teams in NYC H+H primary care and specialty care clinics.
Job Summary:
- Conduct community outreach activities within assigned boroughs of NYC, engaging patients and providing resources and assistance needed to address health issues.
- Motivate and support patients to achieve their family health goals through facilitating connections to primary and specialty care clinics, community services, and mental health support.
- Create individualized, patient-centered care plans to address health, social, and service-related needs of patients.
Key Responsibilities:
- Participate in ongoing education and specialized training to maintain CHW skills and public health emergency response skills.
- Engage eligible patients in clinical or community-based assessments and interviews to identify their health goals and needs.
- Assess and address health, social, and service-related needs of assigned/identified patients and create individualized care plans.
- Support and motivate patients to achieve their health goals by coaching them through behavior change and identifying strengths and community support systems.
- Help patients connect to primary care providers, specialty care providers, community services, and mental health services, attend medical appointments, and fill prescriptions.
Requirements:
- One year of full-time experience in a government agency or community organization in providing community services to the public or assisting members of the community in obtaining community services.
- Familiarity with email, word processing, and Internet usage and comfort learning new computer programs.
- Excellent communication and documentation skills.
- Ability to conduct home visits and outreach activities within assigned boroughs of NYC.
