Job Summary

Sea Mar Community Health Centers is seeking a skilled Community Resource Navigator to join our team. As a key member of our Managed Care Department, you will play a vital role in providing information and assistance to individuals seeking health coverage, healthcare, and other local resources that promote a healthy lifestyle.

The ideal candidate will possess excellent organizational and time-management skills, with the ability to work independently and as part of a team. Strong communication and problem-solving skills are essential, as well as the ability to maintain confidentiality and work in a fast-paced environment.

Responsibilities will include:

  • Providing information and assistance to individuals seeking health coverage and healthcare
  • Helping individuals navigate health insurance options and public programs
  • Assisting with the application process and providing support to newly assigned and high-risk patients
  • Providing case management and intervention strategies to remove barriers to healthcare access

Requirements include:

  • AA or BA in Social Services or Business Administration preferred
  • At least 2 years of customer service experience, preferably in a healthcare environment
  • Marketing and sales experience considered a plus
  • Bilingual in English/Spanish preferred
