Job Summary

Sea Mar Community Health Centers is seeking a skilled Community Resource Navigator to join our Managed Care Department in Vancouver, WA. This full-time position requires a flexible schedule and involves traveling throughout the southwest Washington region.

The Community Resource Navigator will provide information and assistance to individuals seeking health coverage, healthcare, and other local resources. They will educate eligible individuals on how to navigate health insurance options, public programs, and Sea Mar services, and assist with application processes. Additionally, they will aid newly assigned and high-risk patients in scheduling their initial appointment for medical, dental, and care management services.

The ideal candidate will possess excellent organizational and time-management skills, good judgment, and the ability to maintain confidentiality. They will also be proficient in public speaking, have good spelling and grammar, and be able to read and interpret complex documents and reports.

Key Responsibilities:

  • Provide information and assistance to individuals seeking health coverage and healthcare
  • Education on health insurance options, public programs, and Sea Mar services
  • Assist with application processes
  • Aid newly assigned and high-risk patients in scheduling initial appointments
  • Case management and intervention strategies to remove barriers to appointment attendance

Requirements:

  • AA or BA in Social Services or Business Administration preferred
  • At least 2 years of customer service experience required, preferably in a healthcare environment
