Overview

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Enhabit Home Health & Hospice. As a national leader in home-based care, we are committed to providing high-quality patient care in the comfort of their own homes.

As a LPN, you will play a vital role in delivering compassionate and personalized care to our patients. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Key Responsibilities:

  • Provide nursing care to patients in their homes under the direction of RNs and in compliance with physicians' orders.
  • Conduct thorough assessments of patients' physical and emotional needs.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination.
  • Communicate effectively with patients, families, and healthcare team members.

Requirements:

  • Current licensure as a Licensed Practical Nurse in the state of employment.
  • Valid state driver's license.
  • Automobile liability insurance.
  • Dependable transportation kept in good working condition.
  • Ability to drive an automobile in all types of weather conditions.
