LPN Care Coordinator at Stillwater Family Care

- This is a full-time role, requiring 40 hours of work each week.
- The clinic operates from 8 am to 5 pm, Monday through Friday.
- Flexibility to arrive by 7 am when necessary is expected.
Collaborate with physicians, staff, and other healthcare professionals to deliver patient-centered medical home services and coordinate care across the healthcare spectrum for all patients within the practice. The LPN Care Coordinator plays a crucial role in ensuring safety, adhering to best practices, and maintaining high-quality care standards throughout the continuum. Responsibilities include coordinating a variety of self-management support and disease registry initiatives for the patient population. Success will be evaluated based on the process and outcome performance metrics of the patient population and the achievement of relevant healthcare milestones.
Qualifications:
- Graduated from an accredited nursing program.
- Licensed to practice nursing in the state of Oklahoma.
- Effective communication skills, both verbally and in writing.
- Current CPR certification or completion within 30 days of hire.
- Ability to sustain prolonged periods of walking and standing, while managing stressful situations.
- Vision must be adequate, or correctable with glasses, for reading and writing tasks.
- Ability to hear, or correctable with hearing devices, and to engage in conversations in person and over the phone.
- Must be able to hear, see, and feel to accurately assess, monitor, and provide care.
- Capability to frequently lift at least 60 lbs., and perform actions such as reaching, pulling, pushing, bending, stooping, and kneeling.
- Possess fine motor skills and hand-eye coordination necessary for delivering care (e.g., inserting catheters, administering medications, and handling instruments).
