- Provide Education and Training: Provide education and training to direct care team members on best practices, new technologies, and other relevant topics.
- Collaborate with Interdisciplinary Teams: Collaborate with interdisciplinary teams, including physicians, therapists, and other healthcare professionals, to ensure seamless care coordination and communication.
- Education: Associate's degree in a nursing-related field required.
- License: Current state license as a registered nurse required.
- Experience: 2 years of home care experience as a registered nurse in a home health or hospice environment required. 1 year of supervisory experience required.
- Skills: Advanced nursing skills, including ability to manage patient care episodes, changes to plan of care, clinical evaluations, and recommendations. Ability to provide constructive feedback and develop clinical quality, core competencies, and productivity with direct reports.
