Job Summary

Optimus Health Care, Inc. is seeking a highly skilled and compassionate OB/GYN Patient Navigator to join our team in Stamford, CT. As a key member of our healthcare team, you will play a vital role in promoting the Patient-Centered Medical Home (PCMH+) model by serving as a liaison between obstetrics and gynecology, behavioral health, home, and community.

Key Responsibilities
  • Develop and maintain close relationships with OBGYN patients and care team members.
  • Contact patients to assist in setting up obstetric appointments and provide reminder telephone calls prior to every appointment.
  • Complete assessments of social determinants of health and identify resources to overcome barriers to care.
  • Communicate with patients and care team members to facilitate care and set goals, including specialty scheduling based on patient needs.
  • Maintain accurate patient records, schedules, files, and forms.
  • Track patient attendance at OBGYN appointments and initiate outreach and missed appointment procedures as necessary.
  • Resolve insurance concerns and issues by connecting patients with appropriate resources and team members.
  • Assist patients in connecting with necessary supportive care services, including behavioral health and community health workers.
  • Provide culturally sensitive services to patients from diverse backgrounds.
  • Document all encounters and interventions in Epic.
Requirements
  • High School Diploma or GED required; Associate's Degree in health and/or human services or related field preferred.
  • Experience working with ethnically, culturally, and racially diverse populations and community-based programs.
  • Excellent interpersonal skills, including active listening, empathy, and verbal and written etiquette.
  • Bilingual (Spanish and English) required.
