Job Title: OB/GYN Patient Navigator

Optimus Health Care, Inc. is a Federally Qualified Health Care Center with multiple locations in Connecticut. We are a leading provider of primary health care services in Fairfield County, committed to serving all patients regardless of their ability to pay.

We are currently seeking a full-time OB/GYN Patient Navigator to join our team in Stamford, CT. This position is 100% grant funded.

The OB/GYN Patient Navigator will serve as a liaison between obstetrics and gynecology, behavioral health, home, and community. Placed in an outpatient OBGYN setting at various locations, when required. The Patient Navigator will provide individualized comprehensive case management that supports, improves, and maintains overall health and wellbeing of patients served. The Patient Navigator works closely with the care team when applicable, social services agencies, and other community partners to assist with coordination of care and services. Must be comfortable working with culturally diverse populations. Highly personable, very organized, self-directed with reliable transportation. Must be open to working a flexible schedule when needed, coupled with strong verbal and written communication, in addition to a strong commitment to maintaining a high level of confidentiality.

The OB/GYN Patient Navigator will help patients communicate with their healthcare team to get the information needed to make health care decisions. The Patient Navigator will engage patients, create a trusting relationship, navigate patients to Optimus Health Care obstetric appointments, and closely communicate with the patient's entire care team, based on clearly outlined goals and identified needs. The Patient Navigator will aid patients in the coordination and completion of appointments inside Optimus Health Care and help decrease barriers to timely obstetric care. The Patient Navigator will link pregnant patients to Optimus Health Care pediatric care teams.

Essential Functions & Responsibilities


• Develops and maintains close relationships with OBGYN patients, as well as all care team members.


• Contacts patients over the phone or in person to assist in setting up obstetric appointments at Optimus Health Care.


• Provides reminder telephone calls prior to every appointment.


• Completes the prenatal online notification form within 14 days of the first prenatal visit, as required by the State of Connecticut Obstetrics Pay for Performance.


• Assures compliance with Prenatal Measures, including first prenatal visit and risk identification within 14 days of a confirmed pregnancy.


• Completes an assessment of social determinants of health.


• Communicates with patients and the care team members to facilitate care and set goals, including specialty scheduling based on the patient's identified needs.


• Identify resources for patients to overcome barriers to care, such as transportation, housing, and childcare arrangements.


• Maintains accurate patient records, schedules, files, and forms, as needed.


• Track patient attendance at OBGYN appointments and patient navigation sessions and initiate outreach and missed appointment procedures, as necessary.


• Resolves any relevant insurance concerns or issues by connecting patients with appropriate resources and team members.


• Assists patients in connecting with necessary supportive care services, including behavioral health and our Community Health Workers.


• Assists patients in connecting with the care team about available services and resources to support participation, including integrative medicine and pediatric services, when appropriate.


• Provides culturally sensitive services to patients from different cultures.


• Provides referral to external services when issues are identified (i.e., hunger issues, domestic violence issues, etc.).


• Provides advocacy, patient education, and support in accessing Optimus-based, community-based, and hospital-based programs.


• Documents all encounters/interventions in Epic.


• Utilizes our Language Services Program, via Martti, when communicating with patients that speak another language.


• Produces mid-year and end of the year reports on program activities, compiling data from databases and writing up case examples.


• Participates in the evaluation and research activities in the program, as needed.

Job Qualifications/Requirements

EDUCATION: High School Diploma or GED required. Associates Degree in health and/or human services or related field preferred.

EXPERIENCE: Experience working with an ethnically, culturally, and racially diverse office staff and patient population. Previous experience working with community-based programs.

COMMUNICATION SKILLS: Excellent interpersonal skills required, including active listening, ability to show empathy, and appropriate verbal and written etiquette.

LANGUAGES: Bilingual (Spanish and English) is required.
